    Obtain Your Medical Record

    This is needed by your GPXtra doctor to complete your Taxi Medical form.

    Some local councils accept use of your medical summary alongside any specialist clinical letters to complete the assessment; but please check with your council to clarify.

    If you need your full medical record, you can simply request your medical record from your NHS GP.

    Feel free to download and use the support letter below to request your full medical record from your GP if needed.

    Obtain your most recent eye test prescription

What do I need to bring to my Taxi Medical appointment?

Please ensure you bring with you the following to your appointment:

✔ 3. List of medications including dose and start dates

✔ 4. Official Identification such as a driving licence or passport.

✔ 5. Most recent optician prescription.
